***Please note that by submitting this application, you acknowledge that you have read the below indemnification and hold harmless agreement*** 

By applying to use the facilities at Camp Wyanokie, I and all members of my group, agree to indemnify and hold harmless the Camp Wyanokie Commission, the individual Commissioners, and the municipalities who own Camp Wyanokie against and from any and all claims arising from our use of the premises or from any activity, work or other things done, on or about the Premises, and shall further indemnify and hold the aforementioned harmless against and from any and all claims arising from any breach or default in the performance of any obligation on our part to be performed under the terms of this agreement, or arising from any act or negligence, by myself, my guest(s) or invitee of ours, and from all costs, attorney’s fees and liabilities incurred in or about the defense of any such claim or any action or proceeding brought thereon, and in case any action or proceeding be brought against Camp Wyanokie by reason of such claim. I, my group, guests, and invitees hereby assume all risk of damage to Camp Wyanokie, our property or injury or illness to persons in, upon or about the Premises, from ANY cause including the unintentional negligence of the Camp Wyanokie Commission, Commissioners and the municipalities who own Camp Wyanokie.  I also agree to give prompt written notice to Camp Wyanokie in case of damage, casualty, illness or accidents that occur on the premises during my stay.
